KMHC Long Term Care (LTC) is a permanent residence dedicated to providing a multi-disciplinary approach to caring for Kahnawakeronon when they are no longer able to live at home independently. Our team is comprised of clinical and non-clinical staff who offer care to the residents 24/7. Incorporating a cultural approach to ensure the best quality of care and living is offered to the residents. Family involvement is highly encouraged to help the LTC staff get to know the resident and throughout their stay at KMHC

The Admissions –Discharge Process at KMHC follows our Policy. An admission for LTC is usually recommended to the Admissions Committee by either their family physician, home care case manager or external hospitals. For more information, please request to speak to the manager of LTC by calling 450-638-3930, dial 0 and the Operator will transfer your call.
For people who can no longer live at home, and who meet the criteria of policy. Contact family doctor to start the process
RESPITE BED
There is one respite bed in LTC which is available to community members still living at home with a primary caregiver. This is available 1 to 2 weeks at a time to enable the caregiver time to have a break from daily care. They are admitted by the Short Term Care physician. Criteria is based similarly to LTC admission.
24/7 care is provided, the care plan is implemented based on information from family. If the patient is on the waiting list for LTC, it can give them familiarity to help transition to placement.
Their physician must make the initial referral, the patient must be stable as this is not an acute care admission.
Fees are equivalent to LTC residents but are charged a daily rate. television with cable is provided 450-638-3930 dial 0 and ask for the LTC social worker on 2nd floor to enquire about availability
It is recommended that you bring:

- Clothing / Atahkwénnia
- Personal Hygiene Products / Teiontia’tawiróhare’táhkwa’shón:’a
Patients are welcome to bring items that comfort them, like special blankets, pillows, photos, stuffed animals, etc.
- Please note no powders are allowed.
- All electrical appliances (for example: radios and fans) must be checked and approved by Plant Services before being used in the patient’s room.
- Televisions are supplied by resident/family and family must contact the cable company in order to activate the cable. You need to supply the cable company with your name, resident’s room number, and date of admission.
Each resident at KMHC has the comfort and privacy of their own room. With one of the best maintained hospital facilities in the region, KMHC offers a clean and home-like environment for each resident.
