Planning your stay in Short term Care

The mission and philosophy of the Inpatient Care Short Term Care (STC) is to provide the highest level of care using a multidisciplinary approach. A short-term care admission is the hospitalization of an individual for evaluation and/or treatment, for a period of days, up to several weeks (typically not more than 45 days), depending on clinical criteria. Presently, we have ten Short Term Care beds which will increase to fifteen beds in the near future.

We work closely with Turtle bay elders lodge and homecare to discharge patients safely back to their home environment.

Our multidisciplinary team consists of: one physiotherapist, one occupational therapist, one social worker, nurses, PAB’s .

There are twenty-one permanent employees that work in Short Term Care; ten are Kahnawake:ro;non. Short Term Care staffing include; one Manager, eight Registered Nurses, five PABs, two Ward Clerks, one Social Worker, one Volunteer Coordinator, and Occupational Therapist. We are presently still looking to fill out full-time physiotherapist position.

The Admissions –Discharge Process at KMHC follows the recommendations of the rounding physician(s) of the client. For more information, please request to speak to the manager of the desired ward (STC ) by calling 450-638-3930, dial 0 and the Operator will transfer your call.

 

Admissions process STC : If the client is at home their family physician must refer them to STC. If the client is hospitalized at another institution, the treating physician there must reach out to our rounding physician in order to be added to our waitlist.

It is recommended that you bring:

  • Medicare card (RAMQ) / ata’karitátshera aohiatónhsera
  • List of medications and dispill/ sanonhkwa’shón:’a tsi nikahiatónnion
  • Clothing / Atahkwénnia
  • Personal Hygiene Products / Teiontia’tawiróhare’táhkwa’shón:’a
  • List of medications and medications taken at home.

Clients are welcome to bring items that comfort them, like special blankets, pillows, photos, stuffed animals, etc. *

  • Please note no powders are allowed.
  • All electrical appliances (for example: radios and fans) must be checked and approved by Plant Services before being used in the patient’s room.
  • STC Televisions are available in every bedroom. It is your responsibility to contact the cable company in order to activate the cable. You need to supply the cable company with your name, room number, and date of admission.
