Risk Management

The Risk Management’s mandate is to develop and ensure to maintain Patient Safety Culture throughout the organization.

This is further supported by an objective reporting structure which promotes a ‘’no name, no blame, no shame’’ culture. When an incident or accident involves a patient, it is required to complete an AH-223 report, a Ministerial form developed for this purpose. If necessary, an investigation and root cause analysis occurs and recommendations are made by the Quality and Risk Management Team to support the service area involved. 

Reporting allows the team to identify issues and gaps, and relatedly allows for preventative measures to be taken before accidents happen.
