Kahnawake Elders Survey: Healthcare Experiences

KMHC would like to hear from our Elders about their experiences accessing healthcare services within and external to Kahnawake.

The survey has 5 healthcare-related questions and should take approximately 10 minutes to complete.

All responses are confidential and will be used to improve services for our Elders.
